Passport Services:
This service provides directions and guidelines on how to apply for a United States Passport Book.
- We accept the form DS-11 Application for a U.S. Passport
- We don’t process the form DS-82 U.S. Passport Renewal Application, which is a Mail-In only application.

Passport service hours are subject to change without notice.
NOTE: There are limited parking spaces at the Takoma Park Community Center because of the Library Renovation Project. Please allow yourself extra time to find a parking space to keep your appointment time.

How To Apply for a United States Passport Book
To expedite the passport execution process, have the following items ready/complete when you arrive at the Community Center for passport execution services. Failure to provide a completed application and the necessary documents listed below will result in significant processing delays and/or denial of your application.
Regular passport applications take approximately 6-8 weeks to process. Expedited passport applications take 2-3 weeks. The City of Takoma Park has no control over passport processing times.
Applications have to have no errors or mistakes at all. If there is even one error, a new application must be completed.
What to Bring
DS-11 form. This form must be completed and submitted for your application. The form is available at the Community Center or here Complete your DS-11 in black ink. Do not sign your forms before you meet with the Passport Agent.
Original evidence of United States citizenship.
All customers applying for a passport using form DS-11 must submit a clear copy of their citizenship evidence in addition to their original or certified copy of U.S. citizenship. If there is writing on both sides of the citizenship evidence, then copies must be made of the front and back of the document.
The photocopy is required of primary U.S. citizenship evidence such as a certified birth certificate, naturalization certificate, consular report of birth abroad, and certificate of citizenship. Customers who submit an expired U.S. passport book or passport card as citizenship evidence do not need to submit a photocopy.
Photocopies:
- Must be legible
- Must be on white 8.5″ x 11″ standard paper
- Must be copied using black ink
- Cannot be double-sided
Customers who do not submit a photocopy or second certified copy of their citizenship evidence may experience a delay in the processing of their passport application.
Valid photo identification. Acceptable formats:
- Current Government ID (City, Federal, or State)
- Current Military ID (Military and Dependents)
- Driver’s License
- Naturalization Certificate
A photocopy submission of the identification document(s) presented. Details:
- Photocopy must be on plain white 8 1/2″ x 11″ standard paper stock showing the front and back of your ID.
- Photocopy must contain images on only one-side of each paper submitted.
One 2″ x 2″ color passport photo.
- Takoma Park does not take passport photos. You must provide your own.
- Eyeglasses are not allowed in passport photographs
Payment for any applicable fees.
- Passport fees and execution fees must be paid separately. Fees are detailed at the bottom of this page.
- Fees paid to the U.S. Department of State are payable by check or money order only.
- Execution fees paid to the City of Takoma Park can be paid by cash, credit card or money order. There is a convenience fee when paying with a credit card.
Important Passport Information for Applicants
Children
Children under the age of 16 must appear in person with parents. For persons ages 16 and 17, only one parent must appear with the applicant. Passports issued to individuals under the age of 16 cannot be renewed.
Original Documents
All original documents are sent to the U.S. Department of State for review with your application. You will get your original documents in the mail a few days after you receive your passport.

U.S. Passport Books
Ages 16 and older:
- $130.00 passport fee. Payable to U.S. Department of State. Fees paid to the U.S. Department of State are payable by check or money order only.
- $35.00 execution fee. Payable to the City of Takoma Park by cash, credit card*, money order or cashiers check.
Under age 16:
- $100.00 passport fee. Payable to U.S. Department of State. Fees paid to the U.S. Department of State are payable by check or money order only.

U.S. Passport Card
The U.S. Department of State has developed a Passport Card as a more portable and less expensive alternative to the traditional passport book. The New Passport Card can only be used for land and sea travel. The City of Takoma Park will accept applications for the card.
Age 16 and older:
- $30.00 passport fee. Payable to U.S. Department of State. Fees paid to the U.S. Department of State are payable by check or money order only.
- $35.00 execution fee. Payable to the City of Takoma Park by cash, credit card*, money order or cashiers check.
Under age 16:
- $15.00 passport fee. Payable to U.S. Department of State. Fees paid to the U.S. Department of State are payable by check or money order only.

- $150.00 file search fee. Payable to the U.S. Department of State.
Overnight Service
The cost for overnight service from the Department of State to the applicant is $21.36.
